Job Title: Care Coordinator

Care Coordinators play a vital role in assisting patients with managing their overall health. They are responsible for daily monitoring and calls to assigned groups of patients.

Key Responsibilities:

• Daily review of patient recorded vital readings via phone conversations
• Educating patients on improving diagnosed diseases and supporting the plan of care established by their provider
• Verifying and recording patient demographics, medical issues, medications, and risk management factors using specialized medical software
• Handling potentially difficult conversations calmly and professionally

Requirements:

• Strong communication skills to engage patients in conversation and motivate them to follow the care plan
• Able to make a positive impression on patients through every interaction, contributing to a positive attitude with the team and individual goals
